NUCL 154 Admissions, Discharges and Transfers

NUCL 154 Admissions, Discharges and Transfers introduces the learner to the Nursing Unit Clerk's (NUC) role in admission, discharge and transfer procedures within the hospital. Information is given about antibiotic resistant organisms and isolation procedures, the Admitting/Registration Department and their role in admitting patients, and how the NUC interacts with this department. Using case study examples and actual forms, the learner creates an admission chart, a transfer chart and a discharge chart.

Prerequisites: NUCL 152 with a minimum grade of 80%

Upon successful completion of this course, the learner will be able to:

  1. Describe the Admitting/Registration Department’s role in admissions
  2. State how the patient census and bed utilization is tracked in the hospital
  3. Describe the two types of admissions to hospitals
  4. Identify the forms received from the Admitting Department for an admission chart
  5. State the principle behind universal blood and body fluid (BBF) precautions
  6. Define the two basic types of isolation
  7. Describe the NUC’s responsibilities when a patient is put on isolation
  8. Define abbreviations used for antibiotic resistant organisms
  9. State the reasons why a patient may transferred to another unit
  10. State the reasons why a patient may be transferred to a different facility
  11. Describe the duties of the NUC in admitting, discharging and transferring patients
  12. Create and dismantle a mock patient chart
  13. Prepare a mock patient chart for a discharge by transfer and a discharge by death
  14. State the NUC’s role in processing discharge orders

Teaching and Learning Approach

Online students are responsible for creating and following their own study schedule. Instructors are available by phone during scheduled office hours for one-on-one support. Emails may be sent at any time and will be answered during office hours.

Students are expected to:

Complete all graded components according to the due dates listed in the online course calendar.

Participate in the online discussion forum with classmates on the topic of discharging a patient through transfer to another facility.

Create a mock chart for each assignment, using the information provided throughout the course.

Complete a mid-course exam covering content from the first half of the course.

Complete a final exam covering the entire course, with a focus on material from the second half.
